Purchase of Service Program Provider Application Notification

Connections Area Agency on Aging, Inc. is currently accepting applications for our FY 2022 Purchase of Service Program.  Connections’ Purchase of Service Program is aimed at providing a choice for frail adults to receive needed services in the community.   The Purchase of Service Program, as a funder of last resort, will directly purchase needed services for consumers from a pool of competing providers with the choice of provider made by consumers.   

 

Provider Purchase of Service Applications must be returned to the Connections office in Council Bluffs by 5:00 p.m. on June 1, 2021.

 

Applications are available to download on our website, www.connectionsaaa.org, or by phoning 800-432-9209 ext. 8117 to have one mailed to you.

 

Please feel free to make additional copies of the application if you need to.  Also, if the application does not apply to you, feel free to pass it on to someone it may apply to.
