Service Navigator

Service Navigators respond to requests for assistance from individuals with short term and long term concerns.  At the consumer’s direction, the Service Navigator will provide person centered needs assessments, counseling, planning and referral.  For individuals requesting the service, the Service Navigator will help develop a personalized plan of care to meet the identified needs using available home and community based services.  Service Navigator will work to promote the Connections Plus program with consumers, providers and the community.  The Service Navigator will facilitate all projects and programs of the Iowa Family Caregivers Program and the Elder Rights Specialist.

 

Education and Experience: Bachelor’s degree in human service field; or currently licensed Registered Nurse.  Can substitute up to two years of full-time work experience involving direct contact with people in overcoming social, economic, psychological or health problems in a human service field, for two years of the educational requirements.

Must of experience working with the public. Experience in working with the elderly and disabled and knowledge of community resources is a strong plus.

Must have reliable transportation, valid driver’s license, auto insurance, and be able to travel in and outside of service area.  Must successfully pass a Criminal and Dependent Adult Abuse background checks before hire.

This is a full-time position with benefits and will work from either our Council Bluffs, Creston or Sioux City Office.

HELP WANTED

 

Part-time Meal site Manager in the Sioux City area – Monday through Friday approximately 15 hours a week.  Responsible for serving nutritious meals to seniors, clean up and do paperwork.  Must be able to lift and/or move moderately heavy items.  Applications will be available at Connections Area Agency on Aging main office 2301 Pierce St Sioux City and online at ConnectionsAAA.org   Nutrition Assistant Director 712-279-6900 x8225 Application deadline April 5, 2024, EOE
