Going to the hospital for either a planned or unplanned stay for your-self or a loved one can be stressful, especially in our senior years. Coming home from the hospital isn't a walk in the park either. Connections Area Agency on Aging will lead a discussion with local professionals that centers on tips for getting ready for a hospitalization and recovering at home. Learn about community supports available to help you be successful in transitioning home from the hospital or a skilled nursing facility. Caregivers, you don't need to navigate this alone. We will also share information about specialists who are available specifically for the caregiver.

What Information Do We Collect?
