Care Transitions Coach

Care Transitions Coaches ensure the safe and effective transfers in the movement of patients across the care continuum, serving as the bridge between professional staff in the hospital and nursing home setting and the patient and/or family. Acts as a patient educator-advocate empowerment facilitator. Provides information and guidance to the patient and/or family for an effective care transition, improved self-management skills, and enhanced patient-doctor communication.

Requirements:

Currently licensed nurse or degree in social work,

Must have reliable transportation, valid driver's license, auto insurance and must be able to travel in and outside of service area.

Must be able to pass criminal background and dependent adult Abuse background checks before hire.

This position will be based out of the Sioux City office.

PT Resident Manager

Resident manager needed for 40 unit Senior Apartment Complex in Sioux City.

1 bedroom apartment provided plus hourly salary. Provided apartment is on the premises and is a condition of employment. Duties include: collecting rents, monthly newsletter, coordination of social activities, cleaning, showing apartments, etc. Computer experience preferred. Graduation from high school/GED required.

Housing Service Coordinator

Responsibilities:

The coordinator will consult with tenant organizations and resident management operations where appropriate. A Service Coordinator will perform annual assessments and reassessments of tenant situations as needed. Responsible for linking tenants withing the complexes to service providers within the general community.(i.e. case management, personal assistance, homemaker, transportation, counseling, preventative health screening/wellness, legal advocacy, etc.)

Qualifications:

A bachelor's degree in the human service field. Preferable degrees in Social Work Gerontology, Psychology or Counseling. Two years of full time experience involving direct contact with people in overcoming social, economic, psychological, or health problems in human service field can be substituted for the two year of the educations requirement. Must have reliable transportation, valid drivers license and auto insurance. Be able to travel as required for continuing educational requirements and successfully pass a criminal and dependent adult abuse background check before hire.

Skills:

Demonstrate working knowledge of supportive services and other resources for senior citizens in teh area served by the projects. Demonstrate ability to advocate, organize, problem solve and provide results for the elderly. Work cooperatively with public and present self in a professional manner.
