Care Transition Coach

Care Transition Coaches will assist individuals leaving a hospital or nursing facility to transition safely back to the community. The coach will provide person centered counseling to the individuals and will work closely with the facility and community providers. The job will also involve the marketing and education of the program to community providers within a four county region.Travel is required within the four county regions.

Job requirements:

License: Must be an RN or have degree in Human Service field

This position is full-time with benefits including IPERS. One position is available and will be located in our Council Bluffs Office.

Housing Service Coordinator

Job Responsibilities:

The Coordinator will consult with tenant organizations and resident management corporations where appropriate. A Service Coordinator will perform annual assessments and reassessments of tenants' situations as needed. Responsible for linking tenants within the complexes to service providers withing the general community. (i.e. case management, personal assistance, homemaker, transportation, counseling, preventative health screening/wellness, legal advocacy, etc.)

Qualifications:

Bachelor's degree in the human service field. Preferable degrees are social work, gerontology, psychology, or counseling. Two years of full time work experience involving direct contact with people overcoming social, economic, psychological, or health problems in a human service field can be substituted for the educational requirement. Must have reliable transportation, valid driver's license, and auto insurance. Be able to travel as required for continuing education requirements. Must be able to pass a criminal and dependent adult abuse background check before hire.

Skills:

Demonstrate working knowledge of supportive services and other resources for senior citizens in the area served by the complexes. Demonstrate the ability to advocate, organize, problem solve, and provide results for the elderly. Work cooperatively with public and present self in a professional manner.

This is a Part time position with IPERS benefits.

PT Resident Manager

Resident manager needed for 55 unit senior-designated apartment complex. Onsite 1 bedroom apartment provided plus hourly salary.

Duties include: collecting rents, monthly newsletter, coordination of social activities, cleaning, showing apartments, etc. Computer experience preferred.

Graduation from high school/GED required.
